# 131. ¿Cómo se trata la disnea al final de la vida?

En el paciente naive de opioide se inicia a **dosis de 3mg por vía endovenosa o subcutánea** y reevaluación a los 30 minutos. Si **existe mejoría** del síntoma se pauta **perfusión de 15mg** de morfina en **24 horas**. En caso contrario, se administra **perfusión de 30mg de morfina** y emplearemos dosis de rescate de 5mg, si existen episodios agudos de disnea, ajustando la dosis de morfina diaria total en función de las dosis extra precisadas a los largo de 24 horas. Si no mejora, habría que plantear sedación paliativa.
